What you need to know before you schedule an appointment

Ageless Acupuncture is open Tuesday-Saturday from 11am to 6pm. For specific days and times, please refer to our online scheduling. During this time, all patients are asked to wear a face mask for the entire duration of treatment and presence in the clinic. During your first visit, please be prepared to spend 75-90 minutes for the initial consultation and treatment. Wearing comfortable and loose clothing is also recommended. You are free to bring in lab/imaging and any other information that you see important to your visits.

Before your first visit, please identify if you want to use your health insurance benefit and if it will cover the cost of treatment. You can simply call your insurance and inquire about your benefits. For your convenience you can refer to this Insurance Verification Form to ask the right questions from your insurance company.  This form is available for you to fill out electronically once you schedule your appointment and finish filling out the on-boarding forms. We prefer that all patient forms be filled out online through the online portal where you schedule your appointment. Schedule here!

You will be able to review all forms including cancellation form, consent form, and HIPPA form within the appointment confirmation email. For your convenience you can find a short summary to the appointment cancellation policy below. (During COVID-19, no appointment cancelations fees will be collected, the policy below is for your information in the future)

If you need to cancel your appointment within less then 24 hours, there is a charge of $50 for late cancellation. If you happen to miss your appointment without notifying the office we do charge the full cost of the treatment. To avoid having these charges please inform the office if your schedule changes prior to 24 hours of your treatment time. This ensure that other patients are able to come in. We greatly appreciate your understanding on this subject!
